Projecting an Access database onto a PDF form

Currently I have an access database full of client info. I am looking to project this info directly onto various insurance applications, all of which are in PDF format. It is imperative that the applications maintain their original dimensions and that the fields in the database line up with the appropriate places on the applications (i.e. the last name field lines up with the last name position on the application). Is there any software available that can do this conversion or is this even a feasible endeavor?
