Please could somebody help me as i have to setup a project management skills audit questionaire. It is to see the skills level of a project manager , project co-ordinator,project administrator.it is for people already on the job .It must measure their knowlwdge ,skills. attitude. Each may be different questionaires. thanks please help if possible

Perhaps you can "borrow" or "rephrase" some of the sample questions used for PMI certification. At least its a good start.

As with all projects: involve the SMEs -- in this case, the top PMs, co-ordinators and administrators in your organziation. Skills identification and evaluation will vary from organization to organization and highly regarded skills in one company are unlikely to be universally highly regarded.
As the simplest of examples: proficiency in MS Project. If you aren't using it then it's a useless skill. In a large organization using it, a PM may need little experience in some areas (such as day-to-day progress tracking because that's handled by other processes in the organization) and substantial experience in others (add-on functionality purchased from a third-party organization). That skills/experience balance is unlikely to matched by other organizations.
PMs in an organization with a higher level in the capability maturity model will have/need different skill sets than those at an organization that is at a lower level.
