Prohibit Design Refresh Or Replace To Modify on Folders

Good Morning.
I know there's tons of stuff around that tells me how to fix this problem on folders (recently had an issue where users were losing folders after a template upgrade), but what I would like to know is how do some users manage to create some of their folders unprotected? There seems to be no pattern.

Any suggestions would be much appreciated.

There probably is no pattern. Protecting from design refresh/replace is a property that can be changed in the Design client, or at creation time. The user only has to uncheck one thing too many and the folder will be updated/erased at the next design refresh.
