problem with sending form

hi, I have created a form...

when you send the form, instead of directing you to the "thankyou" page simply lists the form submission results...

any ideas what i have done wrong...

if you fill out the form I have disabled the validation for the credit card number - the rest of required fields need some input - dont worry the info will only go to me, just type in generic crap...

Does the script

have a redirect to the "thankyou" page? If not, it will run the script and return to the calling page.
