Problem with MoH on BCM 50 with 4 cabinets

I have a system that has a MoH issue and am stumped as to why.
The MoH is not heard when the call is put "On Hold" and as of recently some callers are saying that they can hear the page why they are on hold listeniing to dead air.
Any ideas?

You didn't say so I have to ask...

Did you program the system? choices are Silence, Tones or Music.
And for Music you have to choose what the source will be.
