problem installing 5.0.7 on HP ML570

I am trying to install 5.0.7 on an old ML570 g2
the system has a 6402 controller with 12 18gig HD's configured as a single 100gig logical drive using a raid 10 configuration. I am trying to install using the 6/10/05 CD downloaded from SCO and the ciss driver from the BTLD disk from HP efs 7.790a.
I have updated all hardware firmware using the latest firmware CD (v7.91) from HP
I have run the hardward diagnostic program through multiple loops from the HP smartStart CD version 7.30 and all checked out OK.
My problem is that the installation runs up to the point when I believe that the installation program is trying to create my file systems (boot/root/swap/u) when it terminates with the error "system error 5: device transfer read failure"

what is going wrong?

Have you tried a different copy of the installation media?
There is also some mention of compatibility between use of a large SWAP area and the USB driver (pretty bizarre combination) here:

I know I've installed 5.0.7 using the ciss driver, but I'm not certain it was an ML5xx server.
