Problem in notification

Hello everybody,

I am working on servicedesk 6.0 installed on win2k. I have some problem notification. When I sent a survey on mail, and when I click on the link provided in the mail, directly survey form opens. In the same way when a call initiation notification is sent, and when I click on the link, I want it to directly open the request description.

Can anyone help me out in this.
