Problem answering Line 2 on a CICS

I have a client that has a VoIP modem servicing the dial. Line 1 rolls to Line 2 and then to Line 3: as it is suppose to. However, just recently the phone rings on Line 2 the attendant tries to pick up the call and just looses the call. The caller hears a busy signal.
Is this a Line Port issue? We called the VoIP provider and they assure me that the port on the VoIP modem is operational and has rollover.
Any suggestions

I would swap the mod cords on the modem, line 2 with 3 and see if issue stays on line 2 of the set.
Sounds like the line card except that the caller hears a busy signal instead of RNA or dead air so then maybe the modem in this case.
