printing fields

I am trying to print out all of the documents that I have created with the name of the form field showing.

I have tried a multitude of ways but I am getting nowhere fast.

anyone know how to do this. I realy don't want to have to write them all in.

I have looked in the help doccumentation for this, but lets face it that is worse than useless.
