Print Different Footers on Multiple Copies

Hi All, New to Adobe hope someone can help.
Using Adobe 5 I created a 1 page form, problem is I need to print 4 copies of completed form, with a different agency name at the bottom of each copy for routing purposes.
Any help appreciated

You might try using a form field for your footer and duplicate it so that it appears on all pages. You could then alter the name in the form field.

Seems to me that Acrobat 6 might allow you to add/manipulate headers and footers. But I could be wrong.
