Primary Key and Index... What is the difference ?

I'm sure that this is an easy one for most of you so... Could you please explain (in simple English) the difference between a Primary Key and an Index and how these are used ?

a primary key is a logical thing

an index is a physical thing

a primary key is used so that foreign keys have something to refer to

an index is used to improve query performance
