I have a Base VCM64v2 module. Incoming calls work fine. When a call is going to fail, I do not hear ringing.

The called party can answer, but the calling party has silence.
AT&T says their flex team sees everything fine on their end.

This may or may not help but I just had some very weird problems for a week or two on an IPO with a PRI; it wasn't audio
problems but a lot of other static and issues, and after fighting diagnosing it for a week, I went there, and unplugged the
AT&T equipment myself and left it for a few minutes, then plugged it back up. Problem solved. AT&T had certified remotely
that everything was good twice.

Yeah, I can't tell you how many times I've solved a "no problem with our equipment, it must be you" issue by rebooting the providers router/gateway/boxthingy.
Though, you did not specify, are you using IP Phones?
