PRI not coming up

I was hoping that a second set of eyes could look at these settings from the telco and let me know if these look correct for an Option 11:

If this is a new circuit, please ensure the carrier has released the Bchannels and the Dchannel. If new circuit in US, this is normally the last step of the turnup.

The circuit is turned up. If I unplug the PRI from the Option 11 and plug it in to an Avaya IP Office, it comes right up and works fine.
 
If you are going from the Opt 11 to an IPO then you are probably using a crossover cable? Are you using a straight through cable going to the Telco?
