I am creating a proposal for a client to switch from Analog to PRI lines. The service provider will provide a block of 12 lines so in order to use all 12 I would need to add an 8ch PRI license correct? Does anyone know the current ballpark price on that license?

The licences are for 2,8 or 22 extra channels so you'd need 2 x 2 to give 12 in total. Search on part 383092 is you want an idea of price.

It looks as though 8 is about 50% more than 2x2 so that may be an option if you are ever likely to need more.
 
Depending on the Avaya IP office version, you wont' be able to get additional PRI licenses without upgrading to R11. If the call traffic is small enough that you will only use 8 channels, then you can just add the card and be done. Even though the provider is giving you 12 channels, you will only be able to use 8 at a time.
