PRI - DTA030 and Remote Alarm OS Threshold 2

Hi All,
I am trying to determine if one of my DTI/PRI cards is faulty.
I have disabled and enabled loop 6 below a number of times without success, and been in contact with our telco vendor. The telco has completed a loop back test on the T1 without any issues.
The card shows an active green light with the yellow light flashing intermittently - no red light at all. All other cards are active green with green DCH channels where applicable.

Below is a print out of LD 60 and LD 96. I am hoping someone might have some ideas on what the problem is:

If I could get some help on changing the OS limit I might be able to get the channels enabled. I tried manually enabling the channels, but that causes the ports to ring each channel.

Flip 5 & 6 or 6 & 7 from the telco into the card, see if problem stays on card or follows PSTN input.

You should test all of your connections to your smart jack. Unplug your cable at the smart jack and put a loop back plug in. Your card should go green but have wrong signaling. If that happens, it's not your problem! Have your carrier test and replace the smart jack.

I will first switch T1's 6 & 7. I have found a spare DTI card to test as well... not sure if it is a previously bad card though.
All your thoughts are very much appreciated!!
