Press 6 in voicemail, get high-low tone

I have a client with IP Office 4.1(9) and whenever anyone presses 6 either in the middle or at the end of listening to a voicemail message (to forward the voicemail to an ext.) the person hears a high-low tone on the phone.

I was thinking some shortcut that is interfering, or part of the phone config. but I am not finding it. Anyone run into this?

Better upgrade to 4.2.11 or 4.2.4, these are more stable then 4.1.9.

Hmmm.. nothing I could do programmatically? Is 4.1(9) really fubar'ed?
