Power issues

I have a customer site where I've had to replace/repair the BCM50 4 times in the last year (2 system failures and 2 HD failures). The original install was a year ago.
The customer has had uneven power issues in the past but insists those issues have been cleared up with their power provider.Yet I've had 2 failures after those issues have supposedly been addressed.
The BCM is on a UPS and shares the same UPS with their Windows server. The windows server has not experienced any issues related to power to my knowledge, nor has any thing else related to telecom or IT.
Is there something I can do to mitigate the possibility of this happening again or some kind of power monitoring tool I can use to get evidence of power issues.
 
We had something similar with a customer
check supply AC:
ground vs live...110~127 VCA
ground vs neutral...MUST be less to 1 VCA
neutral vs live...110~127 VCA

Should have mentioned this in my original post.
The loop start trunks are being delivered over a T1 with a Cisco IAD, so I'm thinking it's highly unlikely the trunks are the source of the problem. Although I guess it couldn't hurt to add a protector anyway and totally eliminate it as a possibility.
