Postfix milter error - Service Unvailable

We have set-up a sendmail milter. It works fine except for a few Clients who uses there customised email script to send mail. It sends few mails then it throws the error - Service Unvailable and then it sends the other mails in the queue.

A few random mails are not send.

What could be the cause of the problem and how it can be resolved.
