Pop-Up Calendar for Access Forms problem

I have added a pop-up calendar for users to use when entering date fields on a form.

Most of the users are able to use this function with no problems.

I have one user when he tries to enter a new record into the database when he clicks on the drop down box for the calendar, he states that the calendar shows up blank and when he tries to pick a date, it will kick him out of the Access Application without the record being added.

Can someone give me some idea on what I need to look for to prevent this from happening?

I instructed him to try on another computer to see if he gets the same results and I am currently waiting to hear from him on this.

Help!

 
Additional information:

I had the user to enter a record without inputting the approval data, close the record and then open the record to add the approval data.

When he performs the above, the calendar still displays blank days but when he clicks on a date it inputs the data in the field and updates the approval fields.

Noone else appears to be experiencing this problem.
 
You might want to check for missing references to the calendar control.
