Polycom / RJ11 to RJ45 wiring, punch down

We need assistance with wiring for a new phone. We have an Avaya Switch, dual S8700s running CM 2.1

Here’s the situation:

We’re adding an analog Polycom phone. This is already configured in the switch, with an analog port allocated.

There is a spare RJ45 jack available at the desired destination, which runs to the server room.

We’d like to be able to plug in the RJ11 conference phone cable (from the Polycom) into the RJ45 jack. Is this possible?

In the wiring closet, we’ve punched down the CAT5 cable in the same order as the other punchdowns on the 110 block: White/Blue Blue, White/Orange Orange, White/Green Green, White/Brown Brown.

We do not know the correct order in which to punch down the pairs on the 110 block, assuming we can use the RJ11 cable on the Polycom phone.

Any assistance is appreciated!
