Please Help... Editing problem

I am helping my teacher to edit his book and I want to put on the odd pages: number of the page followed by his name on the top left.
in the even pages: the name of the book followed by the page number on the top right.

I went to "Add Headers and Footers" under Document and I can only insert the name or the number but not both in the same line.

Does any of you know how could I do this?

I would add page # with Header & Footers aligned right on even pgs only. Then add name of book with Touch up text tool before pg # and copy and paste on all even pages. Save document. Re-open and repeat steps for odd pgs aligned left. Be sure to allow enough margin when doing left side so you don't push text off the page. Its not pretty, but it will do the job.
