Place a line of text after email addresses in a submit form

I wish to place "Subject=Enquiry sent ...." on upload after the email address which is submitted on a form. The script being used is

Once the form is submitted I wish to have that sting of text placed after the email address. Currently I am typing each one which is getting very boring and wasting my time.

Any suggestions would be great.
