Pivot table forms

I was wondering if it is possible to change the look of a form that is generated using the pivot table wizard.

I can get the data etc that I want but I want to change the look of it. E.g the fields are all yes/no fields. I would prefer say to have them look like an option box where if it is filled it is yes instead of having both the true and false options.


It seems as though whatever I do in the design view of the form has no effect on the form view of the form.

Any assistance would be appreciated.
