phones randomly resetting to out of box configuration

I have a MICS with 7.1 software and a PRI. In the past 6 months or so different phones reset their buttons to the factory settings. Odd thing is they keep their system programming. This morning had a phone where all the buttons had reset to a T7316 config and the phone is a T7316e. The alarms list shows problems with the DTI/PRI but no telephone alarms

What is the history? was it R1T1 and upgraded or was it fresh install to 7.1?

This system was brand new install in june 2008. All new wiring from the pole to the phones.
