Here is a simple form created on Word 2003. You will notice that on the Speed Dial lines you can enter description that is left aligned and you can also include on the same line but typing from right the actual phone number for reference and it will be right aligned.
Form has 3 columns that when printed can be folded into 3 parts.
I have included up to 41 extensions but by using dual sided printing you could extend this to more and also the personal individual speed dial numbers 80-99 could be then placed there.
