Phone hearing MOH on all lines

Anyone heard of this: One of our users reports that he gets music on hold no matter what line he tries to select. Looked at the set programming, all looks OK there. Tried turning the music off with F#86 with no results. ANy ideas of where to look next? Thanks in advance.

Yes, I've seen it before.
CICS 6.1 with NEW 7316E phones.
We could even lift the handset and the music would play in the handset.
Only when then phone was ringing would it stop.
And then resume when it stopped ringing.

We replaced the phone.

Then we put the "bad" phone on older system and it has worked fine since.
