perl and apache

I am trying to use a scrip I have written, A usere enters something on a form and then PERL is supposed to opet what was entered in a browser, only I get the following message

Internal Server Error

The server encountered an internal error or misconfiguration and was unable to complete your request.

Please contact the server administrator, skottieb@hotmail.com and inform them of the time the error occurred, and anything you might have done that may have caused the error.

the following line appears in the error log
