Peachtree Classic V13 PDQ errors

Last month out of the blue we started to receive an error (12) message when we tried to run any of our custom written PDQ reports. The problem persisted so I reloaded Peachtree and moved the datafiles to another computer.

Now we are starting to get some reports to work but they are not working correctly. Despite having the fields selected properly in custom reports the output is wrong. We select the name field and get data from another field.

What could be the problem?

We are using Windows XP Pro and Peachtree Classic Accounting Ver 13 (DOS).
 
Usually there are two numbers that appear for a PDQ error.

For an error with 12 being the first number and -2 the second number:

PDQ encountered an end of file marker in the data dictionary. The data dictionary file is damaged.
Restore the files back onto your disk from your most recent backup.

For an error with 12 being the first number and -3 the second number:

PDQ has tried to create a work file and has fun out of disk space or is unable to write to the disk.
Unnecessary files should be removed from the disk to free up disk space.
