Passwords gone vm down

I got a bcm400 4.0 the passwords dissappeared over the weekend nothing works including defaults telset comes up as denied in admin the vm comes up blank i.e feature983 nothing comesup it says nothing just a blank screen not inactive. the bft leds are good green and I have a red over the power. Any ideas anyone? Thanks in advance for any clues. on a Monday no less

the defaults come up as denied in admin i.e 266344 738662 266344 same with fearture 9*8 they also have ivr loaded on there I am wondering if it is the hdd?

You say Bcm 4.0 but you're trying "266344 738662 266344" and feature 9*8 which are for Bcm50 and Bcm450.
Do voicemail still working?
Remove the faceplate and hook up a monitor and see if the hard drive boots up properly.

The above codes are for the 400 4.0 as well. The VM came up after several reboots but was unaccessable. the IVR had only one prompt working. Any way it was the HD THAT HAD AN UNTIMELY PASSING. I replaced and restored and all was well in BCM land.

These BCM'S sure do go thru alot of hard drives it is good to have a couple on hand.
