Passing information from a form

Hi,

I'm not sure if I'm posting in the right forum, but anyway...!!! We use Frontpage to create our Intranet here at work, which is hosted on an IIS server. On the same network, we have a Lotus Notes server. What we would like to do is create a form in Lotus Notes (or even a pop up box), in which users enter their first initial and then their last name (eg jsmith). We would like to then pass this string which would open a URL in the form of "

http://10.0.0.4/mail/"string".nsf?OpenDatabase".

Is this possible??

Sorry, we don't want to create the form in Notes, but rather in FrontPage!!
