PageMaker dropping my Alligators (indexing problem)

I'm having a doozy of a problem with creating an index in PageMaker. I've created several indices before, and have not encountered this problem. I'm hoping I can get some help with this.

I'm using PM 6.5.2 on a G4, with 256MB allocated to the application, which should be more than enough, though I could allocated up to about 700MB if necessary.

I have 19 files assembled in a book publication. The total page count is 688 pages, and there are many many index markers throughout the text. When I go to generate the index (Utilities --> Create Index)it creates an incomplete index (I first noticed this because the entry "Alligator" was not in my index). I figure about 30-40% of my index entries are ignored by the Create Index process.

When I go to Utilities-->Show Index, all index entries are listed along with their correct page. So I know PM is picking up all the index markers, it's just not placing them all.

Has anybody encountered this before, and if so, is there a fix for this? Any help would be appreciated, and please do not hesitate to e-mail or post to me if you need firther information.
