Packaged application gives blank screen

I have a small application I am trying to deploy in Access 2K with MOD SR-1 installed. Two tables, 3 forms, and one report. Seems to package properly and install properly on clean machine. However, when launched on the clean machine, the only thing that appears is a gray screen with a windows flag icon in the top left corner. Any help would be greatly appreciated.
