Package system-release is not installed

I've got a BCM 400 Rel 4.0 that I can't log into with Element Manager. Telset, voicemail, web page, BCM Monitor, and serial port are all OK. If you click on validated device in EM, it says "device validated successfully, device type is BCM 400."

What I've noticed is that on the screen in EM underneath where you put the user name and password it says inventory information. Underneath that are 3 headings-system name, which shows the correct system name, system description, which shows BCM 400 and system software version, which shows "package system-release is not installed."

Has anyone ever seen this before? Any idea what the problem is or how to correct it?

Any help would be greatly appreciated.

I would start by uninstalling element manager, downloading the element manager from that bcm and install that version.
 
Have you tried both LAN ports? I've never come across this problem.
 
In case anyone is interested: Nortel had to log in to the Linux root prompt and reload a whole bunch of files to make this work again. Something like 120 files were lost/corrupt. The engineer that I worked with said that he has run into this issue about a half a dozen times and that they have no idea what the cause is.
