package needs updating?

Why are some of the InDesign Packages I'm receiving firing up the modified links window on opening the file. The info doc is saying everything is fine in each case, but some packages trigger the warning other do not. Surely if they have been packaged, every thing will be in place and linked to the files in the package folder and should open directly?
 
...they might well be linked to the correct image in the correct location, however images can always be modified after they have been packaged from indesign...

These are packages that are complete, with an instruction doc that confirms that all links are present, with none modified and none missing and the package has not been subsequently tampered with in any way, beyond being sent across a network.

When opening the file contained in the package, some open directly with all links intact, others trigger the warning that it contains files that have been modified.

This is the bit I don't understand, if the file has been packaged and all the images gathered and the links updated to reflect their new location within the package, shouldn't every file open without the need to re-link?
