OWA with SBS 4.5 IIS 4.0 and Exchange 5.5 SP 4

I have the problem of not being able to connect from the web to the OWA. It works fine within the LAN, but not outside. I have the required ports open on the router and my firewall (80, 389), but am still not getting any response from the web. I am not even getting the logon screen - page not available. Permissions?? Not sure on the permissions for the Exchange admin, what to do and how to do it. I have a group set up with the logon locally right given to it (local or global)? Any ideas would be appreciated.
