OWA Default page Under Construction

We have been running OWA on a front end server successfully for some time. Last night the IIS crashed three times without obvious explanation. After restarting the services we were able to access OWA remotely only by typing the full address (servername.domain.edu/exchange vs servername.domain.edu). There is a default.htm file, and the default Web server is the Exchange implementation. Any ideas 1) why we lost it and 2) how to get it back? BTW I tried using a redirect vs. local file but it didn't solve the problem.
 
Why did it crash? Was it a drwatson? If so call us and send the user.dmp file and we will give you a hotfix for free for the IIS crashing.

What does it say when you use the other address? Any error? What page comes up?
