OK, this is a little complicated:

Goal: I want to email an invitation to a large number of people outside my firm. I want them to click on the supplied link and have a form pop up with 3 fields: Name, phone # and other comments. In addition, I want the "To:" field completed with my email address and the subject line filled out with "Invitation Reply".

Right now my link just pops up the typical email reply window, but users have to remember to type all the necessary information in the text window.

This invitation will not be sent by me directly to all the invitees... it has to go to another individual first who will forwarded it to all the invitees.

Outlook Forms:
I have tried creating a custom form to do this in Outlook 2003. After creating the form, I made sure to save it after checking "Send form definition with item" and "Use form only for responses" on the (Properties) tab.

I know I could have a webpage created with a hyperlink to accomplish the same thing, but really want to do this solely through email. Some concerns/questions I have are...

1) Am I on the right track?
2) Do I create the form under "Edit Compose Page" or "Edit Read Page"? And what is the difference?
3) When I save the form it is hidden. When I try to access it in "Personal Forms Library" it does not appear, yet when I "Publish As" to the "Personal Forms Library" is appears as "invitation form <hidden>". How do I unhide it?
4) How do I integrate it into the email?
5) Does this generate some kind of VBA script?
6) Will the email be blocked by spam filters due to the form?
