Outlook Form - Missing Text

I have a created the Outlook form as I need it...

I have seven text boxes which have combination values (text and numbers), but when the form is filled out and sent all that comes through is the form...all values are missing.

Granted I only designed the look I'm sure I'm missing what should be under the hood...

Can someone help?
