Outlook form Help

I wanted to create an Outlook form for bug reporting purposes. I have created the form and am able to see it on my screen. How do I activate it for the entire company?

Also I want this form to be hooked up to a database so that I can store all the information. Please advise on if and how to do this.

for all kinds of stuff on Outlook. Only thing I know about what you want to do is that you have to "publish" the form. You'll want to be careful if you publish it so that users can see everything or just data. That's all I remember...
