Outlook Custom Task Form / Access Link

I created a database and linked my Tasks from Outlook to Access utilizing a custom form for my tasks in outlook. When I link my Task to Access I get the standard fields and not the customized fields I created in my Task Form. The correct number of Tasks created appear, but the custom fields and data do not appear. How do I get my custom Task fields in Outlook to appear in Access?

I've searched high and low for relevent threads but have been unsuccessful in locating a like problem.

Any help in this matter is appreciated.
