Outlook 2003 While you were out phone message

Is there a form anywhere that works like a phone message while you were out sheet. In Groupwise (I know ouch, he said a novell product) there is a phone message form that allows you to send a email like message with fields for who. phone number, message, urgency, and so on, in outlook I can not find one, any ideas.
