Outlook 2003 - Renaming a Form

Hey everyone,

I've just inherited a situation where theres a "form" already existing in \\Public Folders\All Public Folders that only 2 users (myself and reception) can edit while everyone can view. The form is a "daysheet" and represents where everyone is througout the day with contact numbers n so forth blah, blah, blah.......MY QUESTION....

How can I rename this form? the title is the name of the previous administrator and I'm just not all that familiar with custom forms. This frequently modified document is heavily relied upon and I just can't afford to have it screwed up. Any ideas where to start?
