outlook 2000 forms

I am having a hard time creating forms. I got all the details in creating my custom form. However one little problem I have, none of the data that was inserted in the fields, are not sent out. Basically the email comes back with nothing on.

I have created custom fileds and bind them to the correct controls.

Any help would be greatly appreciated.
