Oracle Forms 5 and Controls

I am running Oracle Forms 5.0 and was able successfully add an Activex control. The control interacts well with the OF environment, but when I exit the application I get the following error:

F50DBG32.exe - Application Error (title)

The instruction at "0x02cf9585" referenced memory at "0x02f365a0". The memory could not be "read".
Click ok to terminate the application

When I click OK I get another message similar to this and then the application terminates.

Any solutions?
