Optiplex 745 black screen of death 1

I have installed a ATI FireMV2250 graphic card into the PCI slot. When I switch the machine on the monitor goes into sleep mode and the screen goes black. The orange monitor light then comes on

Any help would be glady recieved

If it's replacing a video adapter that's built into the MB, then make sure the adapter is disabled in BIOS.
If it's replacing a video card, then make sure the card is removed, and its software is uninstalled.
If those have been done or are N/A, then see if the system starts in Safe mode. If not, then the card may be bad.

Make sure that the card is seated correctly in the PCI slot.
