Option 51c keeps initializing

I have changed the IODU/C card and installed the install disk and it boots up until it tries to read the disk and then I get error codes SCSI 21-active shelf has no active CMDU, then SCSI 09-hard drive in CMDU is no responding, then SCSI 16-file system initialization failed, reinstall software from installation disk. I can't find my paper that I wrote on but also got message that said install disk is corrupt or bad, reinsert disk and try again. We found another install disk from a 61c with the same release (key codes are on another disk) and still got the same errors and the switch just keeps rebooting. Not sure if the replacement IODU card is bad also, but the errors saying the install disk is bad is throwing me off.
 
I don't think its the install Floppy Disk, I suspect the Hard Drive on the CMDU, I had a similar fault on a option 61C release 4.5, trying to reinstall, kept getting told the install disk was corrupted, tried numerous different install disks (you can make a new one from the install CD)and eventually figured out the replacement CMDU I'd been sent was faulty. The system tells you the install disk is corrupt because the Drive on the CMDU it points to is faulty, but the system thinks its the install disk sending it the wrong drive info. So my suggestion would be to try a known good CMDU (IODU/C).

So I replaced the IODU/C card and still get the same errors as above. So far have tried 2 different floppy install disks and a copy of one of them and 2 different IODU/C cards. Could it be the NT5D03FB call processor card causing this?
 
The only time I ever had an issue boting from the install disk was when I was replacing a CMDU and the new card had a bad floopy drive in it, but I have never done a 51, only the dual cores like a 61 and 81

It was the IODU/C card, was sent 2 bad units, 3rd time was a charm. Thanks everyone for your help.
