Option 11C Power supply failure 1

I am working on Meridian Option 11C which has one expansion cabinet. I have one spare power supply with it. It's expansion cabinet oftenly goes off, And it power up by replacing the power supply. After few days it again goes off and we again replace it's power supply (replace with power supply which has already failed and replaced by spare one) and it continues. Both power supply (installed in expansion cabinet and spare) are being replaced regularly. What is possible reason of power failure because both power supplies are serviceable.
 
Have you got the standard Option 11c cabinet or the smaller rack mounted one?. Generally speaking, the power units for the standard cabinets are pretty good, but you might have to check the dip switch settings are correct that are found hidden on the top below the grill.

You might also have something in the expansion cabinet that is causing the problem such as a extension card. It might be a good idea to disconnect all the cards and reseat them after checking that it's all clean. Also check that the fibre cable is not bent going to the main cabinet.

I am using standard option 11C(pic attached).
I have shifted almost all cards at main cabinet except xcmc and cut(using only one trunk).
Please guide me about dip switches. These are located on fibre rec card or somewhere else ?

From looking at the photo. you have the standard Option 11 cabinrt and a power unit. If you power down the power unit, you might need to look at the top of the power unit under the grill to find out if you have any hidden dip switches. These switches tend to be used if any Nortel stand by power unit were fitted. You need to check that both the main power unit and the expansion one are set the same as on the main unit.

Both cabinet power supply have same dip switches position.1 is at "ON" position and 2,3 and 4 are at "OFF" position.
 
Are you certain that the power unit is failing?. I've known fibre expansion cabinets to go wrong when the fibre cable starts to go faulty and needs to be replaced.
