OPI II noise and off duty problem

Maybee someone can help resolving this: My customer runs tsw (bc13) on the switch, and have an opi II connected. There is a periodic problem, when the operator answer a call (any type) there sometimes is a loud tone (for one second) which both parts can hear, and after this the opi turns into night posision (there is only one operator). I have tried to restart sw units, reinstall etc. I have not replaced the opi yet, cause to long time for delivery. But maybee there is another cause, and maybee some of you have expirienced a problem like this

I see you are running no service pack, latest servicepack is SP4

as there are many operator related fixes in those service packs, it is advised to upgrade to the latest SP, please ask your service provider to upgrade your system.
