Operator station can't answer calls, no dial tone, phone OK

I am a PBX newbie (new duties with a new job), so please forgive me if this is too trivial an issue for this forum.

We have an NEAX2000 with Dterm Series III phones.

All of a sudden, the phone at our company's operator station is unable to answer incoming calls. It will ring, but lifting the receiver only makes the ring softer.

We are also unable to make any calls from that station, as there is no dial tone after lifting the receiver. I have switched phones, patch cables, and the 4DLCD card in the NEAX2000 with known good ones, but still nothing.

Oddly enough, two of the LED's on the 4DLCD card are constantly blinking. Two other cards have constantly blinking LED's as well. I know the blinking is not normal behavior, but I have no idea what it means or what to do about it.

I will be most grateful for any help you can give me.
 
The Led's that are blinking are not programmed yet.Is the phone not working on that card and what lens is it on.
 
That's odd. The LED's that are blinking are mapped to a LEN that has an extension signed to them. The card belonging to the non-working phone is at PIM1, LT01, has 4 ports, LEN's 0068-0071.

I hope I got the terms correct. I don't know what these things are called--I'm going by what I see in the documentation file: NEAX2000_Command Manual.pdf.

Also of note, if you push a speed dial button with the receiver on the hook, it will dial an inside extension, but not if you lift the receiver off the hook first. Pressing a loop button only results in a fast busy signal.

Correction:

We had all the mappings stored in a spreadsheet for referece, but somehow one of the extensions became unassigned where is was assigned last week. Reassigning it took care of one of the blinking LED's.

So ignore my first 2 sentences in the above post.
 
Problem solved. We called in a tech and he completely reprogrammed the station.

Funny how calling in the professoinals seems to work every time... ;)

Not sure what happened with the Len you ID'd as unassigned.
To assign a Dterm (Digital station) the len should be assigned in command 10 as a FXXX where XXX is the station number. After that the button must be assigned in command 9000 like the following.
9000>XXX,ZZ>FYYY
XXX = station number assinged in CM10 without the F
ZZ = key number 01-16 or 01-08
FYYY = Data assigned to the button.
After assigning buttons need to assign the extension you get dialtone from when the handset or speaker are used. This is done with command 93.
93>XXX>YYY
XXX = station number assinged in CM10 without the F
YYY = station number to give dialtone.
