I have a report that lists scores for each of our patients but I only want the report to show the current or most recent score for each patient. Do I need to create a subreport?

No subreport needed. Insert a summary by patient and use a maximum({datefield},{patient}) summary operation. Hide the details section. Software Support for Macola, Crystal Reports and Goldmine

That will give the last date, but not it's score.

Group by patient and sort by date. Place the date and score fields on the group footer. Then suppress the details and group header. It will have all scores in memory, but will only show the most recent. Ken Hamady, On-site Custom Crystal Reports Training & Consulting
